Often, patients, clinicians, and legal experts are not on the same page when patient harm occurs. This lack of a coordinated effort toward resolution can compromise healing and improvement for all groups involved. While we have a good understanding of what’s not working well, such as a culture of silence after an error occurs, the panelists will discuss actionable, interdisciplinary solutions to improve these well known gaps.

- Recognize how medical errors have historically been handled and why there are such disparities in the way different roles typically respond.
- Consider the impact the current methods of dealing with medical error has on patients, families, and others.
- Discuss the expectations that patients and families should have of healthcare organizations in preventing harm before it occurs and how they can participate in keeping themselves safe.
- Summarize the steps everyone can take to improve communication, transparency and respect for each other in dealing with harm when it occurs.
