Shared Learning Network Poster Presentation

Submit a brief abstract (no more than 300 words) outlining your intervention and outcomes.

If your abstract is accepted, you will be notified via email with information to submit a full poster presentation.

Guidelines for the poster presentation include:

The problem and background
The current state and identified gaps
The intervention that was implemented
The resulting outcomes

All posters should be free of commercial bias and should not include any brand names.

Release and License to Use Materials*
I agree to the release and license to use materials below.
By clicking the button above, I am agreeing to either provide the photographs, videos, quotes, or other materials submitted above or any otherwise provided by me (collectively, the “Materials”) to the Patient Safety Movement Foundation (together with any affiliates, successors, or assigns, the “Foundation”) for the "Shared Learning Network."

I am granting the Foundation the right to use the Materials for the Purposes of the Shared Learning Network and the Images and Recording for any use without further consideration to me for an unlimited time. I further grant the Foundation the right to use the Materials and Images in any media, together, or with other videos, photographs, or other media or materials, including without limitation in books, magazines, photo exhibits, films, videotapes, and any other media or publications that contain the Material or Images and Recordings or any portion of them (this
combination, the “Works”) without further consideration to me.

The Foundation will request my permission before using the Materials for any use not included in the
Purpose and will not use my name or the name of any institution with which I am affiliated in a way that appears to be an endorsement of any product without my express permission.

I release the Foundation and its agents, employees, officers, directors and assigns from any and all claims I or any third party may have, now and in the future, for invasion of privacy, right of publicity, copyright infringement, defamation, moral rights, or any other cause of action arising out of the Uses of the Materials, Images and Recordings, and Works permitted in this document.

I have read this release and license and fully understand its contents.
