We bring together a diverse interdisciplinary group of patient safety experts focused on developing practical solutions to end preventable patient harm.
The Movement
The Patient Safety Movement Foundation takes a holistic approach to reducing preventable patient harm. Our movement brings patients, clinicians, healthcare organizations, payers, and policymakers together to address the complexity of healthcare challenges systematically. We have the know-how to achieve ZERO preventable patient harm, but this can only happen in a healthcare setting that has a culture of safety and implemented the necessary Actionable Evidence-Based Practices.
This issue affects us all, regardless of our gender, age, race, geographic location, and socioeconomic status. People who lack the means and access to quality care are particularly vulnerable to preventable harm in healthcare settings. Yet, we also hear of celebrities with access to state-of-the-art care in top medical facilities being the victim of preventable harm and death.
Among the leading causes of preventable patient harm are medication errors, hospital-acquired infections, sepsis, pressure ulcers, communication errors, hospital-acquired pneumonia, and falls. While patient safety statistics often highlight mortality rates, millions of people worldwide survive a medical error only to have their lives critically altered, if not shortened, from a resulting disability. The damage caused by medical errors also extends to the patient’s family and loved ones, who are often left grieving and adjusting their lives around a tragic outcome.
Eliminating preventable patient harm requires awareness, advocacy, and action. To drive safe care—every time for every patient—we must as a healthcare community and society establish a culture of safety, commit to a culture of transparency, and incorporate evidence-based safety practices. Moreover, developing resilience through challenges, practicing innovative solutions, and advocating for the safety of healthcare workers address the complexity of healthcare systems. Our ultimate goal is to promote humanity and diversity in healthcare.
We cannot hope for ZERO, we must plan and execute.

2022
In January, Dr. Michael Ramsay succeeds Dr. David Mayer as the organization’s Chief Executive Officer. In April, the Patient Safety Movement Foundation holds their first virtual annual summit. In June, Dr. Sanaz Massoumi becomes the new Chief Operating Officer to lead the operation of the foundation.
2021
Key global healthcare leaders, including Dr. Tedros Adhanom Ghebreyesus, Director-General of WHO, join us for our Unite for Safe Care virtual event to celebrate WHO Global Patient Safety Day and to help address patient safety across the globe. Over a dozen patient safety advocates representing the Patient Safety Movement Foundation gather at Freedom Plaza in Washington, DC, to march down Pennsylvania Avenue to the Capitol building to bring awareness to patient safety.
2020
The Patient Safety Movement Foundation launches the #uniteforsafecare campaign in June, honoring the millions of lives lost every year due to preventable medical errors. Over 50 speakers share their heart-wrenching stories of survival and loss via a virtual event on World Patient Safety Day in September. The new initiative encourages our committed hospitals to further integrate Actionable Evidence-Based Practices to create safe and reliable healthcare systems with ZERO preventable harm by 2030.
2019
In January, the annual World Patient Safety, Science & Technology Summit is held in Southern California, featuring distinguished keynote speakers that include President Bill Clinton, Dr. Peter Pronovost, Dr. Steven Scheinman, and Dr. Daniel Cole.
2018
The 6th Annual World Patient Safety, Science & Technology’s Summit is held in London at the invitation of UK’s Secretary of State for Health and Social Care, Rt. Hon. Jeremy Hunt, bringing together stakeholders from 25 countries.
2017
President Bill Clinton is appointed Global Chair at the 5th Annual World Patient Safety, Science & Technology Summit. Featured speakers include Dr. Richard Carmona, Dr. Thomas Zeltner, Rt. Hon. Jeremy Hunt, Dr. Didier Pittet, Senator Sheldon Whitehouse, and Senator Robert Casey Jr.
2016
The annual Summit is renamed the World Patient Safety, Science & Technology Summit. Medtronic signs the Open Data Pledge and becomes a benefactor, granting the Patient Safety Movement Foundation $1 million a year through 2020.
2015
Then Vice President Joe Biden provides the keynote presentation at the 3rd Annual Patient Safety, Science & Technology Summit. President Jimmy Carter and wife Rosalynn Carter become Patient Safety Movement Foundation supporters by offering a special prize to organizations that save many lives.
2014
Hospitals in Canada and Lithuania become the first international organizations to make commitments. The Patient Safety Movement Foundation becomes a 501(c)(3) nonprofit organization.
2013
The Patient Safety Movement Foundation’s goal of ZERO preventable patient deaths is announced at the Clinton Global Initiative by Founder and Chairman Joe Kiani. Intermountain Healthcare becomes the first hospital to make a commitment to ZERO at the evening break. The inaugural Patient Safety, Science & Technology Summit sells out, drawing over 300 attendees.
2012
Joe Kiani, Founder and CEO of Masimo, announces the creation of a Patient Safety, Science & Technology Summit to unify the healthcare ecosystem and develop an action and commitment-oriented approach to eliminating preventable patient death. In June, he travels to Africa with President Bill Clinton, who commits to supporting the mission to achieve ZERO preventable deaths. The Masimo Foundation for Ethics, Innovation, and Competition in Healthcare becomes the founding sponsor of the Patient Safety Movement Foundation. The foundation brings together renowned patient safety experts to create Actionable Evidence-Based Practices.
